  1. Wyoming - 6.33 per 100,000
  2. South Dakota - 5.74 per 100,000
  3. Montana - 5.37 per 100,000
  4. Alaska -4.89 per 100,000
  5. Iowa - 4.47 per 100,000
  6. North Dakota - 3.38 per 100,000
  7. New Hampshire - 3.17 per 100,000
  8. West Virginia - 3.08 per 100,000
  9. Kansas - 2.79 per 100,000
  10. Oklahoma - 2.76 per 100,000
